2008 GP Events Speaker Profiles (In Alphabetical Order)
Dr. Payam C. Ataii
Dr. Payam C. Ataii has been in practice since early 1996. His experience covers a wide range of dental specialties including oral surgery, endondontics, pedodontics, periodontics, cosmetic dentistry, and laser dentistry. He graduated first in his clinical class from Tufts Dental School, one of the top five dental schools in the United States, and he also had a Harvard externship. In addition, he has received many achievement awards, and has been published in numerous dental research journals. Dr. Ataii has been an Invisalign Premier Provider since 2005, and was the winner of the 2007 Invisalign GP Summit. He is renowned for spearheading new methods in laser-aided cosmetic dentistry, as well as for his research on the latest technologies for oral cancer detection. He has also been published in four dental publications, and has received three Top Dentist awards. He has also been featured on numerous TV networks, including ABC, NBC, KNBC, and KTLA. In the end, Dr. Ataii is a champion of his patients’ health and happiness, and he believes that they should be able reap the rewards of modern technology at reasonable fees.
Jeff Behan
Jeff Behan began his career as a marketing communications specialist in 1979. His creative work includes marketing strategy, television, radio and Internet production, sales training and branding. Over his career, he has played an integral role in the development of strategy and implementation of marketing campaigns for a diverse array of clients including; Major Public Utility Companies, Intel Corporation, Tele-Communications, Inc., and Delta Airlines as well as numerous orthodontic and dental companies. He is the principal member of VisionTrust Communications, providing corporate video production, customized ads and communications tools to a wide array of clients throughout the United States, with a major emphasis on Orthodontic practices, including many of the top Invisalign providers in the country.
Dr. David Boschken
Dr. David R. Boschken is one of the world’s most prominent Invisalign providers. In addition to his Invisalign Elite practices in the San Francisco bay area, Dr. Boschken is respected as one of Align’s Alpha doctors. Dr. Boschken has been responsible for training Invisalign doctors since 2001 as a member of Align’s Speakers Bureau, and he has consulted on over 12,000 cases as the chief consultant for Invisalign Consulting Services. In addition to mentoring ICS customers with their cases, Dr. Boschken has developed the largest Invisalign study club in the world and provides assistance to hundreds of dentists nationwide, enabling them to better serve their Invisalign patients.
Dr. Rick Boyd
Dr. Richard W. Boyd, Jr. earned his DDS degree at Baylor College of Dentistry in Dallas, Texas, and his MS degree at St. Louis University in St. Louis, Missouri. Dr. Boyd's undergraduate training was in Biology at the University of Texas at Austin from 1969 to 1973. He is a board-certified orthodontist by the American Board of Orthodontics; the only orthodontic specialty board recognized by the American Dental Association and sponsored by the American Association of Orthodontists. Dr. Boyd is an active member of both of these organizations, and was honored to serve as a former president of the Texas Association of Orthodontists. He is also an active member of the Southwestern Society of Orthodontists, the American Dental Association, the Texas Dental Association and the Greater Houston Dental Society.
Dr. Doug Brandt
Dr. Doug Brandt received his DMD from the University of Saskatchewan and his MS and certificate in orthodontics from the University of Detroit. He has maintained a private practice for the last 23 years, and has a large adult patient base. Involved in orthodontic and occlusion education on both the undergraduate and the graduate levels, he is on the cutting edge of the latest technologies. He is presently working with Align Technology at its Treatment Facility in Costa Rica.
Dr. Jim Day
James Day, DDS, Medical P.C. maintains multiple private dental practices in Arizona. He received his dental degree from Georgetown University and completed a GP residency program in the USAF Dental Corps. He earned his masters degree and did his Oral Medicine Fellowship at the University of Washington in Seattle. Dr. Day is an international speaker for Invisalign, and a Premier Provider with over 150 cases treated. He has extensive experience and training in traditional orthodontics. His background spans over 20 years in private practice (including orthodontics) with faculty positions at the University of Missouri School of Dentistry, the University of Washington School of Dentistry, and Northern Arizona University. His goal is to support practitioners and help them increase prosperity and achieve best practices in their patient-care and business models.
Dr. Kathryn Delfs
Dr. Delfs is a graduate of the University of Michigan, where she earned a BS degree in Chemistry. In 1990 she earned her DDS from Northwestern University Dental School. In 1997, Dr. Delfs was honored by the American Association of Dental Schools as one of the top 20 minority dentists in the United States. Her teaching credits include serving as an associate clinical instructor in the Restorative Department of Northwestern University Dental School, as well as acting as Associate Attending Resident at Michael Reese Hospital in its General Practice Residency program. She currently teaches the downtown Chicago Invisalign Study Club for general practitioners, and was a speaker at the annual Invisalign GP Summit for 2007. Dr. Delfs practices full-time in downtown Chicago with her group, University Associates in Dentistry.
Dr. Jacquie Fulop-Goodling
After Dr. Jacqueline Fulop-Goodling completed her DMD degree and orthodontic residency at Boston University School of Dental Medicine, she assumed a professorship at her alma mater, becoming the first female director of its Predoctoral Orthodontic department. Dr. Fulop-Goodling has instructed over 500 students and 5,000 dentists and their teams in multiple areas of orthodontics. She has several U.S. and international patents issued, two of which are dentistry-related. In September of 2005, Dr. Fulop-Goodling became Invisalign’s national spokesperson for its improved hygiene campaign. She has spent considerable time conducting research, and is a member of Invisalign’s Premier Elite Club, awarded to the top 1% of Invisalign’s providers worldwide.
Dr. Tony Gadbois
Dr. Tony Gadbois is a graduate of the University of Tennessee College of Dentistry. He practices dentistry with his wife in their hometown in central Missouri. Dr. Gadbois is a general dentist who has truly succeeded in total integration of Invisalign into his family dentistry practice. Since becoming an Invisalign provider in 2001, Dr. Gadbois has treated over 300 Invisalign cases. His energy and passion for incorporating Invisalign into his general dentistry practice has made him a much sought-after mentor to other dental professionals. Dr. Edward D. Gardner Jr.
Dr. Edward D. Gardner Jr. received his undergraduate degree in Business Administration from The College of William and Mary and his DDS from the Medical College of Virginia, Virginia Commonwealth University. He has been a practicing general dentist for 33 years, and is an Invisalign Premier Provider with over 200 cases submitted. He has lectured to, conducted seminars for, and consulted dental professionals throughout the United States.
Dr. David Gates
Dr. David Gates has owned a cosmetic, reconstructive, and implant practice in Las Vegas, NV since 1992. He was the first GP to be trained by Invisalign in the state of Nevada in 2001. Dr. Gates lectures throughout the U.S., Canada, and Central America, and has taught over 100 Invisalign courses, and conducted numerous study clubs. He was awarded as the top speaker at both the 2005 and 2006 Invisalign Summit Conferences, and is a graduate from Marquette University.
Dr. Steven Glassman
A graduate of Brandeis University and the Columbia University School of Dental and Oral Surgery, Dr. Steven Glassman owns and operates a thriving New York City cosmetic dental practice with his wife. As an innovator in the field of restorative dentistry, he has published several articles in industry magazines including Inside Dentistry and Contemporary Esthetics Magazine on topics including Invisalign technology and restorative dentistry, implant dentistry and laser dentistry. Dr. Glassman has diligently pursued continuing education in his field, and has worked as an instructor at Columbia University Dental School as an instructor of Nd Yag, Ho Yag, Diode, and Erbium laser. He has also worked as an Invisalign instructor for Align Technology, and has served as a guest lecturer for Discus Dental.
Dr. Adam Goodman
Adam Goodman is a Manhattan-based, board-certified orthodontist and Invisalign Elite provider, placing him in the top 1% of Invisalign providers in the country. He is a former consultant with Invisalign Consulting Services, and co-founder of ClinReview consulting. Dr. Goodman has lectured extensively for Align Technology, and has conducted numerous GP study clubs in the New York area. He considers himself a pioneer in the mentoring of GP Invisalign offices.
Dr. Brian Gray
Dr. Brian Gray is a 1986 graduate of the Georgetown University School of Dentistry. He is a Master in the Academy of General Dentistry, and a Fellow in the International Congress of Oral Implantologists and the Academy of Dentistry International. Dr. Gray is also an instructor at the L.D. Pankey Institute, the Dawson Institute, and seven universities. He is a graduate of the Pacific Aesthetic Continuum and the Pankey Continuum. A number of dental manufacturers and non-profit clinical research facilities rely on his input for product evaluation and research and development. He has trained over 7,000 general practitioners in the Invisalign process and helped develop treatment plans for over 3,000 cases. Dr. Gray maintains a full-time private practice in Washington, D.C.
Dr. Perry Jones
Dr. Perry Jones is a 1974 graduate of Virginia Commonwealth University Dental School, where he is currently an adjunct faculty associate professor in both the Oral Maxillofacial Surgery department and the Oral Pathology department. A general dentist, he is a Fellow of the Academy of General Dentistry and maintains an active private practice in Richmond, Virginia. Also involved in implant and scanning technology, he serves as Clinical Advisor for Advanced Dental Technology. Orthodontics has been a key practice focus for him for some 30 years. Dr. Ray McLendon operates multiple orthodontic offices in Houston, Texas, and is one of the world’s most experienced Invisalign doctors. Dr. Ben Miraglia
Dr. Ben Miraglia, a graduate of the State University at Buffalo School of Dental Medicine, is a member of the ADA, the AGD, the NYSAGD, and the Ninth District Dental Society. He has been in private practice for 14 years, and currently maintains a family practice in Mt. Kisco, New York, with a focus on full-mouth reconstructive dentistry, including Invisalign and implants. Dr. Carlyn Phucas
Dr. Carlyn Phucas has been providing quality orthodontics resulting in beautiful smiles for over 15 years. She attended Virginia Commonwealth University, and the Medical College of Virginia. Following graduation she attended Albert Einstein Medical Center, where she completed her orthodontic residency. Dr. Phucas is an active member of the American Dental Association, the American Association of Orthodontists, and the Middle Atlantic Society of Orthodontics. Dr. Doug Sakurai
Dr. Sakurai was born in Los Angeles and has remained there throughout his life. He has earned a B.S. degree in Biology from CSULA in 1984, and his D.D.S. degree from UCLA in 1988. Dr. Sakurai is an Invisalign Premier Provider, and a member of the American Association of Dentistry, the California Dental Association, and the Academy of General Dentistry. He is past clinical and lecturing instructor at UCLA, and is also a graduate of the Pan-Live Aesthetic Continuum. He currently resides in Orange County with his wife Tiffany (also a dentist in private practice in Torrance) and his dog, George. They enjoy world travel and the outdoors together, and he has climbed to the top of Mt. Kilamanjaro. Dr. Lou Shuman
Dr. Lou Shuman serves as Vice President of Strategic Relations for Align Technology. As past Vice President of Clinical Education, he was responsible for the clinical creation of the company’s GP and orthodontic clinical programs. He was also responsible for integrating the Invisalign technique into the curricula of 36 major dental schools. He has lectured on Invisalign throughout the United States, Canada, Europe, and Japan, and is the professional lead on many upcoming projects at Align. He is the past owner of a ten-doctor private group practice where he specialized in adult orthodontics, and he has an extensive background in general dental practice management. Dr. Trent Smallwood
In 1997 Dr. Trent W. Smallwood founded the Centre for Contemporary Dental Concepts in Tempe, Arizona. Dr. Smallwood is held with the highest regard within the American Academy of Cosmetic Dentistry, and has spoken during its annual meeting for the last several years. Along with consulting and teaching with major dental manufacturers, dental laboratories, and his peers, Dr. Smallwood currently teaches with the prestigious Hornbrook Group (a live patient hands-on course that teaches doctors how to build beautiful smiles on their own patients), and in 2003 was named the co-director of the Advanced Posterior Program within this group. Dr. Smallwood lectures nationally, specializing on topics such as full-mouth rehabilitation, ten-minute multiple aesthetic provisionals and practice management.
Dr. Rene Sterental
Rene Sterental, DDS, is the Clinical Director of New Product Development at Align Technology, where he has worked since February, 2003. He is also a member of Align Technology’s Speakers Bureau, and he was a contributing author of the textbook, “The Invisalign System,” edited by Dr. Orhan C. Tuncay. Additionally, Dr. Sterental is an Adjunct Clinical Assistant Professor in the Invisalign Undergraduate Clinic at the University of the Pacific, San Francisco. He is an international member of the American Association of Orthodontics, and a member of the World Federation of Orthodontics and the Venezuelan Society of Orthodontics. Prior to joining Align Technology, Dr. Sterental worked in private practice as an orthodontist in Caracas, Venezuela, from 1989 to 2002. Before that, he worked in private practice in general dentistry from 1985 to 1989. He earned his dental and orthodontic degrees from Universidad Central de Venezuela, Caracas.
Dr. Rob Van den Berg
Dr. Rob Van den Berg is a specialist in Orthodontics and Dentofacial Orthopedics as recognized by the American Association of Orthodontists. His career in dentistry began in 1986 when he received his dental degree from the Academic Center for Dentistry in Amsterdam. Dr. Van den Berg obtained his Orthodontic Degree and Masters in Oral Biology from the University of California, San Francisco in 1996. Dr. Van den Berg is actively involved with cutting-edge advances in orthodontic technology. He co-authored four issued patents on Invisalign. Dr. Van den Berg has trained several thousand other orthodontists and dentists in both basic and advanced Invisalign concepts. He offers Invisalign to a broad range of patients in his office, including teenagers and patients requiring complex care. Dr. Robert Boyd
Dr. Robert L. Boyd holds degrees in both orthodontics and periodontics from the UniJversity of Pennsylvania. He was formerly professor and Chairman of Orthodontics at the University of California, San Francisco, and is currently a full professor and the Frederick T. West Endowed Chair of Orthodontics at the University of the Pacific in San Francisco. Dr. Boyd treated seven of the first 10 cases that were done with the Invisalign technique starting in 1998. He was also the author of the first published article about Invisalign in the year 2000 in the Journal of Clinical Orthodontics, and has authored seven other Invisalign articles. Between his research patients, his private patients, and his teaching patients, Dr Boyd had handled more than 600 Invisalign cases.
Dr. Clark D. Colville
Dr. Clark Colville earned a certificate and Masters degree from the UT Houston Dental Branch Orthodontic Program in 1993, and he maintains a private practice in Seguin, Texas. He became an Invisalign provider in December 1998 and was involved in the first clinical trials as an Alpha Group member. He is currently on the Invisalign Speakers Bureau, Clinical Advisory Board, and continues in ongoing clinical research. He is an Assistant Clinical Professor in the Department of Orthodontics at UT Houston. Dr. Colville is the AAO Chair on the Council of Membership, Ethics, and Judicial Concerns, a director for the Southwestern Society of Orthodontists, and is the current President of the Texas Association of Orthodontists.
Dr. Willy Dayan
Dr. Dayan received his DDS from the University of Toronto in 1986. Following a year of general practice residency at the Montreal Jewish General Hospital and two years in private general practice, he returned to the University of Toronto and completed his diploma in Orthodontics in 1991. In addition to his full-time private practice in orthodontics, he is a guest lecturer at the University of Toronto Department of Orthodontics, The Toronto Academy of Cosmetic Dentistry, The Ontario Dental Association, and The Alpha Omega Dental Fraternity. Dr. Dayan attends the Pankey Institute for Advanced Dental Education. He became an Invisalign provider in the fall of 1999, has treated over 400 cases, and is currently an Elite Premier Provider.
Dr. Ken Fischer
Dr. Ken Fischer received his Orthodontic Certification from UCLA and has maintained a private practice in Villa Park, California for 32 years. He is a guest lecturer at UCLA and Loma Linda's Orthodontic program. In addition he is a OCSD Forensic Odontologist and has served two terms on the CDA Judicial Council, is a past President of the OCDS, the California Association of Orthodontists, and has served on the PCSO board of directors. He has treated over 670 cases with Invisalign and also serves on Align's Clinical Advisory Board.
Dr. William V. Gierie
Dr. William V. Gierie received his Doctor of Dental Surgery degree and a Certificate in Orthodontics from the University of North Carolina at Chapel Hill. He has been providing quality orthodontics in the Wilmington, North Carolina area since 1998. As part of the Invisalign Speakers Bureau, Dr. Gierie lectures around the country. He also speaks to local study clubs and has served as a guest lecturer in the department of orthodontics at the University of North Carolina. Dr. Jonathan Nicozisis
Dr. Nicozisis has had a private practice in Princeton, NJ since 1999. He earned his DMD at the University of Pennsylvania in 1997, and his MS and certificate in orthodontics from the Temple University in 1999. He is a Diplomate of the ABO and a member of the Angle Society. He trained in Invisalign in 2000, and has treated more than 350 patients . He has taught the Invisalign CEI course, and regional study clubs. He has lectured at past Invisalign Summits, and was recently added to Align’s Speakers Bureau. As the founding orthodontist of BAS Medical (now Corthea), he helped raise two rounds of venture capital funding to investigate the hormone Relaxin’s potential role as a therapeutic for mitigating relapse in orthodontics. At this writing, an initial round of FDA human clinical trials has been completed. He was involved in the development of the Smart Retainer, which monitors patient compliance in retention, and he holds two patents related to the Appliance Removal Tool, which is intended to be used with aligners.
Dr. Robert "Tito" Norris
Dr. Robert "Tito" Norris completed his orthodontic training at Howard University in 1995, and served in the U.S. Air Force before opening his practice in San Antonio, Texas. A Diplomate of the ABO, Dr. Norris has been on the Align Speakers Bureau and its Clinical Advisory Board since their inceptions. He is a seven-time winner of Align's "State Your Case" contest, and a finalist in the 2007 "Invisalign Summit Case Shootout."

Dr. David E. Paquette holds a degree in pediatric dentistry from the University of North Carolina, a degree in Orthodontics from St. Louis University, and is board-certified in both specialties. He has been in private practice in Charlotte, North Carolina since 1989. Dr. Mark J. Perelmuter has had a private practice in orthodontics in Louisville, Kentucky since 1980. He graduated from the University of Louisville Dental School, and received his certification in orthodontics from St. Louis University in 1980. He also serves on the faculty of the Louisville School of Dentistry. Dr. Perlmuter has done extensive research for Align Technology, including studies on the detailing plier, dynamic attachments, and the bonding template. He is affiliated with Align's Alpha Group, its Speakers Bureau, and its Clinical Advisory Board. He has done over 700 Invisalign cases, and has had published articles in JCO, AJO-DO, Praxis, and Straight Talk.
He is also involved with numerous study clubs and in-office training for general practitioners. He has lectured extensively, and has spoken at the Kentucky Dental Association, the University of Louisville, the University of Kentucky, St. Louis University, Indiana University, Vanderbilt University, the University of Florida, and the Hadassah University School of Dentistry in Israel. He has spoken at five Invisalign Summits, and also spoke at the 2008 AAO conference in Denver, CO.
Dr. Chad Smart
Dr. Chad Smart maintains his private orthodontic practice in Tacoma, Washington. He received his dental and orthodontic training from UCLA. He has had five Invisalign cases selected for the “State Your Case” program and was the winner of the first Invisalign Case Shootout in 2005. He has treated over 350 cases to date and is recognized as an Invisalign Premier Provider.
Dr. Randol Womack
Dr. Randol Womack received his dental degree at the University of Missouri, Kansas City. He completed a pedodontic internship at the Eastman Dental Center in Rochester, New York, then entered the orthodontic residency at Eastman Dental Center, graduating in 1966 as a Certified Orthodontic Specialist. Dr. Womack was certified by the American Board of Orthodontics in 1974, and was selected to serve on the Align Technology Clinical Advisory Board in 2005. Since Invisalign was first introduced in 1999, Dr. Womack has worked on more than 500 Invisalign cases, and is involved with Align Technology’s Alpha Group, its Speakers Bureau, Summit/IPW I and II, and its Clinical Advisory Board. Dr. Womack was awarded the Invisalign distinction of "Premier Provider" in 2005 and 2006, and in 2007 was designated as an "Elite" Premier Provider.
